What is not known
Approved for HIV-associated lipodystrophy and studied only in that population. The two approved presentations carry different labelled amounts, which is why the lower figure here is 1.28 mg rather than a round number.

Why it is here
Selective ghrelin receptor agonist, reported to release growth hormone with less effect on cortisol and prolactin than earlier secretagogues.
What is not known
Ipamorelin acetate is on the FDA's active Category 2 list of bulk drug substances that may present significant safety risks, which bars its use in 503A compounding. Its only human randomised trial gave 0.03 mg/kg intravenously twice daily for postoperative ileus, roughly 2 mg per administration for a 70 kg adult, and missed its primary endpoint. The figures above are about a tenth of that mass by a different route, with no trial behind them.

Buying the pre-blended vial and costing it as two compounds
Several vendors sell a single tesamorelin/ipamorelin vial. The ratio inside is the vendor's, not this plan's, and it may not be either compound's studied amount. The cost table below will not substitute a blend for two single compounds, which is why a blend listing does not appear in it.
Expecting tesamorelin's trial result outside its trial population
Every tesamorelin efficacy figure comes from adults with HIV-associated lipodystrophy. Effect size in anyone else is not established, and a 2026 review in the American Journal of Sports Medicine states plainly that it has no supporting orthopaedic evidence.
Reading murine synergy as human synergy
The combination data for GHRH analogues with ghrelin agonists is mouse data. It is a reason to run the trial, not a substitute for having run it.
Ignoring what daily tesamorelin costs
Once daily for twelve weeks is 84 administrations of a 2 mg amount. That is a large amount of material, and the vial arithmetic below is unforgiving.
